2000-050 LBFH Agreement for Water Main Design for Conroy DriveRESOLUTION 50-2000 • A RESOLUTION OF THE VILLAGE COUNCIL OF THE VILLAGE OF NORTH PALM BEACH, FLORIDA, AUTHORIZING LBFH, INC. TO PREPARE THE DESIGN AND CONSTRUCTION PLANS FOR A SIX INCH WATER MAIN EXTENSION ALONG CONROY DRIVE BETWEEN PROSPERITY FARMS ROAD AND ALTERNATE AlA WITHIN THE VILLAGE IN ACCORDANCE WITH THE SCOPE OF SERVICES ATTACHED AS EXHIBIT "A" FOR THE SUM OF FIVE THOUSAND FOUR HUNDRED DOLLARS ($5,400.00); PROVIDING FOR A COMPLETION DATE FOR THE SCOPE OF SERVICES TO BE NINETY (90) DAYS FROM ISSUANCE OF A PURCHASE ORDER BY THE VILLAGE; AND, PROVIDING FOR AN EFFECTIVE DATE. BE IT RESOLVED BY THE VILLAGE COUNCIL OF NORTH PALM BEACH, FLORIDA: Section 1. The Village Council of the Village of North Palm Beach, Florida, does hereby authorize LBFH, Inc. to prepare the design and construction plans for a six inch water main extension along Conroy Drive between Prosperity Farms Road and Alternate AlA within the Village in accordance with the scope of services attached as Exhibit "A" for the sum of Five Thousand Four Hundred ($5,400.00) Dollars. Section 2. LBFH, Inc. is to complete the scope of services attached as Exhibit "A" ninety (90) days from issuance of a purchase order by the Village. Section 3. This Resolution shall take effect immediately upon LBFH, Inc. furnishing the Village with a written document accepting the terms and provisions set forth in this Resolution. (Village Seal) MAYOR • ATTEST: ~%/~~~ VILLAG ERK ~ Exhibit "A" llesien and Construction Plans Preaaration: Discussions with staff of VNPB and SUA to confirm extent of scoped improvements, tie-in locations, routing of the main, appurtenances and Seacoast Utility Authority (SUA) development criteria. Field inspection of site and limits of design. Kick-off meeting with staff. 4. Investigate existing utilities; obtain relative elevations of driveways to be disturbed. 5. Establish specific water line route. 6. The water main design shall include services, conflict resolution with existing drainage piping and connection to existing 6" mains at east and west ends. Assemble SUA standard water system details. Prepare technical specifications and contract documents based on current SUA specifications and VNPB standard front end documents. VNPB will provide assistance with this task. 9. Plan review and red line checking. 10. Prepare 90% plans and submit to VNPB for final review and comment. 11. Prepare a summary of estimated quantities and one opinion of probable construction costs. 12. Receive VNPB final review comments and make final revisions to construction plans (100% plans).
