PUBLIC HEARING AND SECOND READING OF ORDINANCE 2012-08 SIMULATED

GAMBLING DEVICES
Consider a motion to adopt and enact on second reading
Ordinance 2012-08, a proposed ordinance amending Chapter 17 of the Village Code of
Ordinances to prohibit the use of simulated gambling devices within the Village.

The Consent Agenda is for the purpose of expediting issues of a routine or
pro-forma nature. Councilmen may remove any item from the Consent Agenda, which would automatically
convey that item to the Regular Agenda for separate discussion and vote.

E. OTHER VILLAGE BUSINESS MATTERS
RESOLUTION – CIGNA INSURANCE PROPOSAL
1. Consider a motion to adopt a
proposed resolution accepting the proposal submitted by Cigna to provide health, dental,
and other insurance policies to Village employees; and authorizing Village Administration to
take all actions necessary to implement these insurance policies and to cancel policies
currently in effect as of 10/1/12.

RESOLUTION – PARTICIPATION IN FLORIDA PRIME
2. Consider a motion to adopt a
proposed resolution authorizing participation in the Local Government Surplus Funds Trust
Fund (Florida Prime) and execution of the Florida PRIME Disclosure Statement.

3. Consider a motion to adopt a proposed
resolution increasing the fees charged for emergency medical transport services provided
by the Fire Rescue Department.

RESOLUTION – ALLOCATION OF $12.50 FUNDS TO MUNICIPALITIES
4. Consider a
motion to adopt a proposed resolution supporting the County’s continued allocation of
$12.50 Funds to the municipalities pursuant to Palm Beach County Resolution R2002-0192.
